Scheduling
Call (505)299-4414 to schedule your appointment. New patient wait times can be up to 2-3 months. However, you may receive an earlier appointment if you ask to be put on our cancellation list.
Office Hours
Monday-Thursday: 8:30am-12pm, 1:30pm-5pm
Friday: 8:30am-11:30am
Insurance
Most major insurances are accepted. Call for specific insurance information.
Cancellation Policy
Please provide 24 hours notice.
Day of Your Appointment
Please arrive 15 minutes prior to your appointment to complete the new patient registration forms. Print out and complete the form below before your appointment to expedite the registration process.
Please bring your insurance card(s) and a picture ID. Patients will be rescheduled if this information is not brought with them the day of their appointment.
